Step 1: Prepare to file an application.
1. If you want to receive the provision of education to a bank account, get the number of that account.
2. Verify that you should prepare annexes to the application. Depending on your situation, it may be:
– scan documents, which concern the adoption of a child – if you are a parent of an adopted child,
– a scan of a final court judgment of divorce or separation – if you’re after a divorce or separation,
– scan of the ruling family court to establish the child’s legal guardian – if you are a legal guardian,
– a scan of the residence card – if you are a foreigner,
– scan a temporary residence permit in connection with work in a profession that requires high qualifications – if you are a foreigner and you are not a citizen of the European Union, the European Economic Area or Switzerland.

Step 2: Login to the PUE ZUS.

Confirmation will be sent to you from an address: upo500plus@mrpips.gov.pl, and the message will have the title of “Official confirmation of receiving the request for the provision of 500+.” Further investigation will be carried out in writing, and the decision on the benefits you will receive a letter.
